Fees and Scheduling
Following your call, email, or completion of the "Getting Started" form and my follow-up response (phone call or email, based on your preference), you will be provided with an intake packet to complete electronically. It must be received and reviewed before we can schedule an initial consultation.

I am in-network with many Aetna and MedCost plans. If I am listed as in-network in your insurance plan’s provider directory (as “Joshua M Harris” and/or “Six Pines PLLC”) and you would like to use your in-network benefit, please inform me prior to scheduling your intake session so that I may assist you in verifying your benefits. If I am in-network with your plan and we are filing with your insurance, your full co-pay and any required co-insurance (e.g. unmet deductible, sometimes other costs) are due at the time of service. While I will provide you with whatever assistance I can in helping you receive the insurance benefits to which you are entitled, you (not your insurance company) are responsible for full payment of fees.
With the exceptions of Aetna and MedCost, I am out-of-network with all insurance plans and operate on a fee-for-service model. If wanted, I can provide a Super bill for you to submit to your insurance company. The most common billing codes I use are 90791 (intake), and 90837 (53-minute individual therapy session).
The full fee for the intake is $150. Follow-up appointments are $100-150, depending on session type. Payment is due at the time of the visit via credit card or HSA card. If you are seeking services on a self-pay basis or with out-of-network insurance and need a lower fee, please contact me to discuss.
Please Note: I am NOT a Medicare or Medicaid provider. Due to my status as a non-Medicare provider, I am unable to accept persons into my practice who are covered by Medicare (either as primary or secondary). I am also not authorized to provide services to clients who are Medicaid beneficiaries (primary or secondary), regardless of how you might have chosen to fund services.
I am currently a TRICARE Certified, non-network, non-participating provider with TRICARE East. You can verify my status by searching the following terms at the website below:

This status means that plan subscribers and their beneficiaries with TRICARE SELECT through TRICARE EAST are likely eligible for out-of-network reimbursement for medically necessary psychotherapy at my practice. If you are interested in treatment with me at Six Pines PLLC and would like to use your out-of-network TRICARE SELECT through TRICARE East, please contact me regarding my rates (which, due to federal regulations are different than those listed on my treatment agreement) and your likely net out-of-pocket costs (once you are reimbursed by TRICARE/Humana).
